How do I register with Burgess Road Surgery?
To register with the Surgery you may attend in person with your NHS card or
fill in a form at the Surgery reception.
You can also print off the
GMS1 form,
Adult
Registration form,
Child
Registration Form,
and Audit C for
Alcohol here.
Please complete the forms, sign
and come to the surgery with 2 forms of ID, 1 photographic and one with
your current address on it.

How do I find the Surgery?
We are near the Swaythling end of Burgess Road about 500m from the University.
On Burgess Road we are on the opposite side of Burger King restaurant at the
corner of Burgess Road and Tulip Road. The Surgery is next to St Albans Church
with its main entrance on Tulip Road. There is parking for 22 cars with reserved
area for the disabled.

How do I see the Doctor?
Appointments to see the doctor or the nurse can be made in person or by phoning 023 80 676233
Monday to Friday from 8am to 6pm.
You can also book appointments online.

How do I speak to the Doctor by phone?
If you need to speak to the Doctor over the telephone please ask the
Receptionist who will suggest a good time for you to phone or the Doctor can
phone you back.

How do I arrange a home visit?
Home visits are only for people who are housebound or genuinely too ill to
attend the surgery. If you require the doctor to call at your home please phone
the surgery between 8am and 10am so that the doctors can plan their
visits.
Please do not ask the doctor to visit unless the patient is housebound or
genuinely too ill to come to the surgery.

How do I get a repeat prescription?
If you are on regular medication please see the Doctor. The Doctor needs to
authorise the repeat prescription. The Surgery has a computerised repeat
prescription system. Once the Doctor has authorised the repeat prescription you
may obtain repeat prescriptions. Please allow 2 working days for the
prescription to be processed. From time to time you may be asked to see the
Nurse or the Doctor to check on your health.

How do I make a complaint?
If
you have a complaint or concern about the service you have received at this
practice please let us know.
We
operate a Practice Complaints Procedure as part of an NHS system for dealing
with complaints. Our complaints
system meets national criteria.
Most problems and complaints can be sorted out at the time they arise with
the person concerned.
If
your problem cannot be sorted out in this way and you wish to make a complaint,
we would like you to let us know as soon as possible, ideally within a matter of
days or at most a few weeks. If
this is not possible please let us have details of your complaint either
within 6 months of the problem or
within 6 months of discovering that you have a problem and provided this
is within 12 months of the incident.

Complete
the Practice Complaints form and send it to the practice or hand it to the Receptionists.
The
form will be passed to the Practice Manager, Ms Tamzen Hogben, or in her absence
the Senior Receptionist.

Procedure leaflet.
